About AHS (Alexandra Health System)

AHS is a network of medical institutions to serve Singapore residents in the north. Our network comprises Khoo Teck Puat Hospital, Yishun Community Hospital, as well as the following facilities that will be operational progressively from 2017 to 2022: Admiralty Medical Centre, Sembawang Primary Care Centre and Woodlands Health Campus.

Health Status

As a health ecosystem in northern Singapore, we engage the community and residents to take charge of their own health. Our plan of care for patients differ by their individual health status:

For well and healthy residents, we encourage them to keep up their healthy lifestyle. These residents are championed to become a sphere of good influence, and role models for their families and peers.

For residents at risk of developing chronic illnesses, we want to alert and nudge them to take proactive and pre-emptive actions before they fall prey to a chronic disease.

For residents with existing chronic and medical conditions, we work with them to manage their medical conditions and guide them towards taking greater ownership of their health. Our goal is to enable them to lead their desired lifestyles independently and where needed, seek their primary care doctors for follow-up.

For patients who are unwell and unhealthy, we help them to live meaningfully in the community after discharge. Some patients may have multiple medical conditions and are frequently admitted into hospitals. We design intervention programmes to empower this group of patients to live comfortably at home and facilitate ageing in place that will reduce their inconveniences of re-admissions.

For patients who are frail, we hope to bring them and their families some relief, comfort and closure as these patients approach the end of their life journey.
